HCM Access Form The University's enterprise Human Resources/Payroll management application is the PeopleSoft HCM system. It encompasses various functions such as Reporting, Workforce Administration, Personal Details, Time and Labor, Absence Management, and other HR/Payroll related procedures. To request roles related to Human Resources, Payroll (including HCM Distribution), Time & Labor, and Absence Management, please fill out this form and upload to a Service Now ticket.  
W-4 Withholding Form [pdf] This pdf version of the form is only available for international employees. All other employees, please use Employee Self Service in HCM PeopleSoft.  
SC W-4 Withholding Form [pdf] Required to determine the  state tax withholdings deducted from your earnings.
